CONDITIONS OF ENTRY

AT THIS PRACTICE:

All patients (including SYDPATH patients) must adhere to the following Conditions of Entry which are in place for the safety and protection of doctors, staff, and patients.

  • Patients are seen by appointment ONLY.
  • We do NOT see emergencies.
  • We will not accept new patients to the practice who do NOT have an appointment.
  • An appointment can ONLY be made by telephone: 93719474
  • Please DO NOT try and make an appointment through the intercom
  • You will advise us BEFORE ENTRY if you have fever, sore throat, shortness of breath, or cough
  • You agree if requested by the doctors to undergo COVID-19 testing
  • If you have been diagnosed with COVID-19, at least 7 days have elapsed since your diagnosis and your symptoms have resolved, and you have performed a Negative Rapid Antigen test on the day of your anticipated entry.
  • You agree to always wear a mask over your nose and mouth while inside the premises.
  • You agree to always practise social distancing while within the grounds of the practice.
  • You agree to wait outside to meet the requirements of social distancing in the building.
  • You agree to wash your hands or use hand sanitiser on entering the building and as instructed by the doctors or staff.
  • You agree that you will not consume food or drink while inside the premises.

WE REQUEST THAT YOU SHOW DOCTORS AND STAFF THE RESPECT AND PATIENCE YOU EXPECT TO RECEIVE.
DISREPECT, RUDENESS AND AGGRESSIVE BEHAVIOUR WILL NOT BE TOLERATED.
